    Defect
Toyota is recalling certain model year 2010 through 2014 Prius vehicles. In the affected vehicles, the Intelligent Power Module (IPM) inside the inverter module (a component of the hybrid system) contains transistors that may become damaged from high operating temperatures. If this occurs, various warning lamps will be illuminated on the instrument panel and the vehicle will have reduced power allowing it to only drive a short distance.
    Consequence
The vehicle may enter a fail-safe/limp-home mode that limits the drivability of the vehicle. The hybrid system could also shut down completely resulting in a vehicle stall, increasing the risk of a crash.
    Remedy
Toyota will notify owners and dealers will update the software for both the motor/generator control electronic control unit (ECU) and the hybrid control ECU, free of charge. If an owner experiences a failure of the inverter before the vehicle receives updated software, the dealer will repair or replace the inverter assembly with a new one at no charge. Reported July 2, 2013Model years 2008–2009DVSA recall R/2013/070EU Safety Gate A12/0741/139,882 vehicles affected

A fatigue crack in the brake pressure accumulator may let nitrogen enter the brake fluid, lengthening pedal travel and reducing hydraulic pressure, which can increase stopping distance. The brake booster will be checked and replaced with an improved one if needed.

    Official DVSA recall text
    Component
    BRAKING EFFICIENCY MAY BE AFFECTED
    Defect
The subject vehicles are equipped with brake pressure accumulators consisting of a metal plunger containing brake fluid encased in a metal housing. The plunger is designed with metal pleated bellows to allow for motion. Nitrogen gas is sealed between the plunger and housing. It is possible that a fatigue crack could develop in the bellows due to vertical vibration of the plunger while driving. If this occurs nitrogen gas could leak into the brake fluid and gradually cause the brake pedal stroke to become longer resulting in decreased hydraulic pressure which could result in increased stopping distance.
    Remedy
Recall the vehicles that are likely to be affected to first check which type of brake booster is fitted and if the at risk type is fitted; replace the brake booster assembly with an improved one.

Nitrogen gas from a component of the brake booster pump assembly could leak into the brake fluid.
    Consequence
As nitrogen is released into the brake fluid, the brake pedal may have reduced hydraulic pressure. Under certain circumstances, this condition could affect stopping distance and increase the risk of a crash.

These vehicles were sold with labels that were outside the allowable one percent of accuracy of actual weight added. Thus, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 110, "Tire Selection and Rims."
    Consequence
An inaccurate label could lead to owners overloading their vehicles and tires. An overloaded vehicle can result in a tire failure which may result in a vehicle crash, personal injury, or property damage.
    Remedy
Southeast Toyota will notify owners and provide a corrected label with instructions concerning its installation. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Without passing the calibration test, the occupant sensing system may not operate as designed.
    Consequence
If the front passenger seat occupant sensing system is out of calibration, the front passenger airbags may not deploy or they may deploy inappropriately for the passenger's size and position. This could increase the risk of personal injury during the event of a vehicle crash necessitating airbag deployment.

Reported December 3, 2012Model years 2003–2009DVSA recall R/2012/14652,503 vehicles affected

A scratched pump motor coil may corrode or break, stopping coolant circulation; a short circuit could blow a fuse and stop the hybrid system while driving. Dealers will replace the high-voltage electric water pump.

    Official DVSA recall text
    Component
    SHORT CIRCUIT MAY OCCUR
    Defect
In the Hybrid system of the subject vehicles there is an electrically driven water pump assembly that circulates coolant through the hybrid components. It is possible the coil wire of the electric motor installed in the water pump may have been scratched during the coiling manufacturing process at the supplier. In this condition the coil wire may corrode at the scratched portion and in some cases break. If this occurs the water pump could stop working leading to illumination of various warning lights in the instrument panel. In some cases a short circuit occurs between adjacent coil wires resulting in a blown fuse for the electric power supply circuit. This will cause the hybrid system to stop while the vehicle is being driven which increases the risk of an accident.

During manufacturing, a scratch may have occurred inside of the electrically driven water pump at the coil wire. The coil wire may corrode at the scratched portion.
    Consequence
The corroded coil wire may break and the water pump could stop. The corroded coil wire could cause a short circuit between coil wires and a possible open fuse, creating a stall-like condition of the hybrid system while the vehicle is being driven. This may increase the risk of a vehicle crash.
